Options Counseling

Options Counseling is a person-centered process whereby individuals, family members and/or significant others are supported to develop a plan for addressing long term services and supports needs that aligns with their preferences, strengths, values, and needs. Options Counselors 1) conduct a person-centered interview to discover what is important to and important for the individual, help them identify and weigh available options and make decisions, 2) assist in developing a person-centered plan detailing the individual's decisions, immediate next steps and long term objectives, 3) assist the individual to connect with public and privately funded services as needed, and 4) follow-up over time to ensure individuals are meeting their objectives and accessing desired services.

States serve a substantial number of people with dementia and their family caregivers in their Aging Networks and LTSS systems. Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

The purpose of the My Health, My Life Toolkit is to: Address barriers in health care for individuals with I/DD; Improve communication when the individual enters and leaves a hospital or care facility; and Build or strengthen relationships between medical providers and the individual and his or her family or caregiver. It includes tools that allow the individual to keep track of personal health information, the families or guardians to engage in the care planning process and a provider support team to understand the individual’s medical preferences and history.

Florida complete-screening example

This form is for a screening based on Florida's Medicaid and public assistance programs. Answer categories (instead of open-ended answers) make it easier to complete. By entering this client information into its Information and Referral (I&R) software application, the ADRC is able to share the client's responses among the agency staff assisting clients with Medicaid benefits, SNAP (Supplemental Nutrition Assistance Program, formerly Food Stamps), SHINE (Florida's State Health Insurance Program), and the Low Income Subsidy (LIS).

The answers are verified by staff as applications are completed throughout the process and the data can be updated with changes/additions when necessary. This streamlines the application process for the client since each staff member does not have to ask for the information to be repeated. The data collected are also available to the I&R Helpline and Intake staff allowing them to see what transpired, which helps them better serve the client on any subsequent calls.

This training is intended for staff who directly receive and respond to inquiries about long-term care options from consumers and families, although program managers and planners will also benefit. The course defines options counseling within the context of the national ADRC initiative, outlines key competencies needed to offer options counseling, and uses fictional family profiles to illustrate concepts. The Art of Options Counseling content is continually being updated based on national standards development and feedback from training participants. This is the most recent version of the training delivered in Cleveland, OH on August 1, 2011.

This 2002 instructional bulletin from the Minnesota Department of Human Services explains new legislation exempting certain short-term nursing facility admissions from the general preadmission screening requirement. This amendment was intended to reflect the increasing number of very short stays in facilities that occur as part of rehabilitation or subacute care upon hospital discharge.

MN Bulletin on Preadmission Screening Requirement for Nursing Facility Admission (December 23, 2004)

This 2004 instructional bulletin from the Minnesota Department of Human Services explained a new requirement. Preadmission screening is a requirement for nursing facility admission, through long term care consultation. Starting in 2005, nursing facility claims must show that these requirements have been met, or providers may not charge patients for nursing facility stays, regardless of the payer source.
