Community Partners
Aging and Disability Resource Centers and Community Living Programs play an important part in the comprehensive effort to reform long-term care. Such a significant endeavor requires
strategic partnerships at both the state and local levels, as well as solid working agreements among state agencies, local health and human services authorities, service providers, and other private partners. On average, ADRC grantees report having nine formal partnerships at the state level and eight at the pilot site level. Grantees have learned that it is important to involve partners early in the planning process, to set clear and realistic expectations for partnerships, and to formalize partnerships through written agreements or protocols. Grantees have also co-located and cross-trained staff with their partners, conducted joint marketing and outreach campaigns, and collaborated on serving clients.

Making the Long-Term Services and Supports System Work for People with Dementia and Their Caregivers 

Source: TAEStates serve a substantial number of people with dementia and their family caregivers in their Aging Networks and LTSS systems. Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

About the ADD Family Support 360 Initiative
