| The Oklahoma Department of Human Services Aging Services Division (ASD), is working in collaboration with the Alzheimer's Association Oklahoma Chapter (AA), the Tulsa Area on Aging (TAAA), Muscogee (Creek) Nation of Oklahoma, Integris Medical Center, OASIS Adult Day Services, Catholic Charities, and the College of Allied Health of the University of Oklahoma Health Sciences Center. |
| Project Overview and Objectives |
The purpose of this collaborative partnership is to reach persons with dementia (PWD) in the early stages (ES) of Alzheimer's disease and related dementias (ADRD), and their families, to equip and support them, mitigating later crises.
Project objectives: • To establish the COMPASS: Early Stage Directions Program (COMPASS), a system of supportive interventions of the consumer, thereby meeting consumer needs for information and support, and increasing the voice and self-direction of ES PWD. • To build and strengthen relationships among community agencies. • To expand community awareness of ES issues, identify more ES families, and increase access to ES programs. • To evaluate impact of added services. • To disseminate project information.

| Individuals with early stage ADRD and their caregivers will be the target population for this project. |
| Anticipated Outcomes |
| The expected outcomes are for ES PWD and their families to report increased knowledge of the disease and care options, as well as increased ability to cope and plan effectively for future needs as the disease progresses. |
