Program Design
Overall Model Tennessee's ADRCs will serve as a "one-stop", single entry points for providing consumers easy access to information, counseling, assistance, and linkages to a wide range of long-term support services and living options throughout the state. The ADRCs will work with consumers to make informed decisions, and either make arrangements for the services needed, or make connections with the service provider to limit the need for the consumer to go elsewhere or duplicate their efforts. Web site enhancements are planned, including a Web-based resource directory. Tennessee will ensure ADRC concepts and functions are embedded in the state's long term care system by coordinating staff, technology, partnerships, marketing, evaluation and accomplishing the project goals and objectives.
Target Populations Persons age 60+ and adults (age 21+) with physical disabilities
Streamlining Access Plans The intake, assessment and case management for the "Options for Community Living" Program (Options) and the Statewide HCBS Waiver have been conducted at the local level at the 9 Area Agencies on Aging and Disability across the state. The ADRCs will move towards a more coordinated single entry and delivery system for home and community services for the target populations. The ADRCs will also work to improve links with health and social services providers through methods such as improved Web sites, databases, and referral forms. The ADRCs will provider wider access by expanding beyond their physical locations and establishing electronic connections to other areas of the district. Efforts will also focus on co-locating as many key services as possible within the ADRCs, or enabling other agencies' functions to be transmitted electronically to the ADRCs to complete the consumer's intake.
Program Expansion and Sustainability Tennessee plans on building on the existing ADRC project to create fully functioning ADRCs in all nine regions of the state.

Care Transitions As of October 2010, Tennessee, a 2010 AoA Care Transitions grantee, had two ADRCs actively conducting care transitions activities, one indentified using the Care Transitions Intervention.
