Building a Collaborative Partnership between the Money Follows the Person Rebalancing Demonstration and Aging and Disability Resource Centers is aimed at states with both ADRC and MFP programs. This toolkit provides useful information and helpful tools to aid States in coordinating ADRC and MFP programs towards joint goals. Efforts are being made at the Federal and State levels to encourage system change to afford Americans with disabilities greater choice and control. State health and human service programs are struggling with economic and social policy constraints resulting in an ever increasing need for partnership within and across public and private programs.

States serve a substantial number of people with dementia and their family caregivers in their Aging Networks and LTSS systems. Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

My Thinker’s Not Working: A National Strategy for Enabling Adults with Intellectual Disabilities Affected by dementia to Remain in the Community and Receive Quality Supports was developed by a national task group co-chaired by the University of Illinois at Chicago's Research and Training Center on Aging and I/DD and the American Academy of Developmental Medicine and Dentistry. Task group objectives were to identify a workable screening instrument to help assess and substantiate suspicions of dementia-related decline, suggest and promote the development of practice guidelines for post-diagnosis health care and supports, and recommend models of community-based support and long term care for persons with an intellectual disability affected by dementia.

Prior to the Alzheimer’s Disease Supportive Services Program (ADSSP), the Administration on Aging funded the Alzheimer’s Disease Demonstration Grants to States program (ADDGS). This report from June 2008 examines the ways in which seven ADDGS grantees work with the U.S. Administration on Aging-funded National Family Caregiver Support Program (NFCSP) in their respective states to support family caregivers. The seven states are Nevada, New Mexico, Tennessee, Texas, Vermont, Washington, and Wisconsin.

STEADI Fall Prevention Toolkit

A preventive toolkit to help health care providers prevent falls. The great majority of primary care providers are unaware of the American Geriatrics Society's Clinical Practice Guidelines on preventing falls in older persons as well as of standardized fall risk assessment methods. To address these identified gaps the Centers for Disease Control and Prevention's Injury Center developed the STEADI toolkit. This report summarizes results of Alaska's Provider Survey. It examined capacity across the ADRC system in Alaska to provide options counseling. The survey was designed to get an indication of what relevant knowledge and skills people brought when hired how much this grew while working with the ADRC how people learned these skills and where there were systemic gaps that could be addressed by focused training.
