General Partnership Strategies
Aging and Disability Resource Centers and Community Living Programs play an important part in the comprehensive effort to reform long-term care. Such a significant endeavor requires
strategic partnerships at both the state and local levels, as well as solid working agreements among state agencies, local health and human services authorities, service providers, and other private partners. On average, ADRC grantees report having nine formal partnerships at the state level and eight at the pilot site level. Grantees have learned that it is important to involve partners early in the planning process, to set clear and realistic expectations for partnerships, and to formalize partnerships through written agreements or protocols. Grantees have also co-located and cross-trained staff with their partners, conducted joint marketing and outreach campaigns, and collaborated on serving clients.

2012 ADRC Joint Program Official Funding Announcement
Source: ACLThis is a funding opportunity from the Administation for Community Living (ACL), Centers for Medicare and Medicaid Services (CMS), and the Veteran's Health Administration (VHA), number HHS-2012-ACL-RO-1210. This opportunity is for states to significantly strengthen and expand their long term services and supports Options Counseling (LTSS OC) programs. Application deadline is July 25, 2012.

Building a Collaborative Partnership between the Money Follows the Person Rebalancing Demonstration and Aging and Disability Resource Centers is aimed at states with both ADRC and MFP programs. This toolkit provides useful information and helpful tools to aid States in coordinating ADRC and MFP programs towards joint goals. Efforts are being made at the Federal and State levels to encourage system change to afford Americans with disabilities greater choice and control. State health and human service programs are struggling with economic and social policy constraints resulting in an ever increasing need for partnership within and across public and private programs.

Virginia's PCP Terminology Chart

This terminology chart represents work Virginia has done through a CMS Systems Transformation Grant to make terminology in statutes, regulations and policies across the health and human services system more person-centered. They have incorporated this terminology especially into their Money Follows the Person (MFP) program and into draft options counseling standards. The chart is also an example of the Aging & Disability Partnerships Terminology Survey.

A Guide to Memorandum of Understanding Negotiation and Development

Through the use of a Memorandum of Understanding (MOU), agreements between agencies can be formalized. This paper discusses the process an agency should follow to develop interagency MOUs and presents examples of MOUs which can be used as models. It was originally prepared for the agencies participating in the National Long Term Care Channeling Demonstration Program, but has since been widely adapted and disseminated.

NASUA Findings Brief - Ombudsman Coordination with ADRCs

In the spring of 2007, the National Association of State Units on Aging (NASUA) collected data about how ombudsman programs are coordinating with Aging and Disability Resource Centers (ADRCs) by asking state ombudsmen in the 40 states and the District of Columbia with an ADRC grant, to respond to a short web-based questionnaire. This document presents key findings, provides examples of coordination between long-term care ombudsman programs and ADRCs and identifies some of the factors which may influence collaboration between the ombudsman program and ADRCs.
