NC Texas ADRC Nursing Home Relocation Evaluation Report

The North Central Texas Council of Governments’ (NCTCOG’s) Area Agency on Aging—the lead agency for the North Central Texas Aging and Disability Resource Center—has provided nursing home relocation services under a performance-based contract with the Texas Department of Aging and Disability Services (DADS) since 2007. Under this contract NCTCOG provides relocation services to residents who have been deemed eligible for Medicaid and require intense case management in order to transition. This report summarizes results of an evaluation of the program.

Mississippi ADRC Focus Group Report

In May 2008, the Institute for Disability Studies (IDS) conducted three focus groups in the Central Mississippi Planning and Development District / Area Agency on Aging (CMPDD/AAA) service area. It was designed to gather information from individuals with disabilities and older individuals concerning the status of home and community based services and independent living related services supporting individuals’ desire and efforts to maintain independent living status and avoid institutionalization. Discussion was facilitated by IDS and focused on AoA factors impacting successful Aging and Disability Resource Center (ADRC) implementation: visibility and trust, ease of access, responsiveness, and effectiveness.

Hawaii Streamlining and Access Focus Groups Results

It was determined that each focus group should be relatively homogeneous in make-up in order to maximize the potential for shared experiences. Streamlining Access Phone Survey Results

This document summarizes the responses provided by 23 of the 24 cohorts of 2003 and 2004 ADRC grantees during the fall of 2006 regarding streamlining access processes in their states. Grantees' responses covered the process for how financial eligibility determinations are made for Medicaid and other public LTC programs in their state, as well as how level of care assessments are conducted in their states, both before and after the establishment of the ADRC program. Other questions asked included the ADRC's role in nursing facility transitions, provision of formal options counseling to consumers, and work to intervene with critical pathways. The aggregated information provided through the streamlining access surveys has been classified and discussed according to the "fully functioning ADRC" program components and recommended metrics. The survey information collected is very helpful in providing a more detailed understanding of grantees' achievements and progress related to streamlining access.
