Program Sustainability and Expansion Strategies

Grant programs are expected to ensure the sustainability of their core activities after the initial grant funding has ended in order to achieve long-term goals and meaningful system reform. Sustainability requires building partnerships, adopting new business processes, training staff, and using information technology in new and more efficient ways. Achieving sustainability also requires foresight and creative thinking. Early grantees learned that it is important to plan for sustainability of the program at the beginning of the project, to involve consumers and community partners in meaningful ways in program planning so they become invested in the vision and goals of the program; and to view the ADRC and/or CLP as part of the state’s ongoing vision for long-term care systems reform.

We invite you to explore the resources on this page about achieving sustainability. We also encourage you to access the peer-to-peer exchange if you would like to post a question or comment for response from other website users. ADRC Profiles in Sustainability

CMS FY 2008 Aging and Disability Resource Center Grant Solicitation

CMS was awarded $5 million for Aging and Disability Resource Center/Area Agencies on Aging grants for FY08. The FY 2008 Grant Category is "Development and Implementation of a Person-centered Hospital Discharge Planning Model." The new grant solicitation will build upon the successes and direction of earlier Real Choice Systems Change and Aging and Disability Resource Center grant opportunities by providing targeted assistance to States’ in their efforts to improve hospital discharge planning through collaboration with, Aging and Disability Resource Centers (ADRCs), Area Agencies on Aging, and Centers for Independent Living.

Texas’s Use of Administrative Funds to Expand the ADRC

The Texas Department of Aging and Disability Services embarked upon an initiative to use their Older Americans Act administrative funds to expand the Aging and Disability Resource Center program into five new regions. This funding will support these five additional sites for two years beginning in 2008. Texas is the first state to use OAA administrative funding to directly support or expand ADRC activities. Other states with excess un-obligated OAA administrative funds might consider using these funds to support existing ADRCs or expand ADRC services into new regions.

Published Letter About Maryland MAP from County Executive

ADRC's are faced with the challenges of sustainability and ongoing marketing outreach to consumers. Maryland Access Point of Howard County, MD was featured in the local publication SeniorConnection. In each edition the County Executive writes an opening letter about the feature. It is letters like this example that can help the local ADRC work toward the goals of sustainability and outreach.
