Options Counseling

Long-Term Support Options Counseling is an interactive decision-support process whereby consumers, family members and/or significant others are supported in their deliberations to determine appropriate long-term care choices in the context of a consumer’s needs, preferences, values, and individual circumstances. The foundation for options counseling is a robust information and assistance program, so it may not be very different from the type of support that your organization currently provides. However, it is important to keep in mind that options counseling should involve supporting consumers in their own choices by providing unbiased information. Options counselors ensure that consumers have considered a range possibilities when making a decision about long-term supports, and they encourage planning for future needs. Many grantees have reported that providing options counseling is a longer process and takes more time than traditional information and assistance services. Each organization must decide for itself what constitutes options counseling, how it may differ from other information and assistance services provided, and how it will be recorded and reported.

States serve a substantial number of people with dementia and their family caregivers in their Aging Networks and LTSS systems. Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

Florida complete-screening example

This form is for a screening based on Florida's Medicaid and public assistance programs. Answer categories (instead of open-ended answers) make it easier to complete. By entering this client information into its Information and Referral (I&R) software application, the ADRC is able to share the client's responses among the agency staff assisting clients with Medicaid benefits, SNAP (Supplemental Nutrition Assistance Program, formerly Food Stamps), SHINE (Florida's State Health Insurance Program), and the Low Income Subsidy (LIS).

The answers are verified by staff as applications are completed throughout the process and the data can be updated with changes/additions when necessary. This streamlines the application process for the client since each staff member does not have to ask for the information to be repeated. The data collected are also available to the I&R Helpline and Intake staff allowing them to see what transpired, which helps them better serve the client on any subsequent calls.

Preadmission screening is a requirement for nursing facility admission, through long term care consultation.
