Program Design
Overall Model New Jersey now has Aging & Disability Resource Centers in every county in the state. The ADRCs provide a minimum of information and assistance and the Screen for Community Services. The State's Offices of Community Choice Options conducts comprehensive care needs assessment for Medicaid-eligible and those within 180 days of eligibility.
Target Populations Persons age 60+ and adults with physical disabilities
Streamlining Access Plans The Department of Health and Senior Services is working with the state Medicaid agency to redesign the eligibility process. The state Medicaid agency has already drafted a revised eligibility application and produced a Consumer Checklist of Supportive Documentation. The ADRC Sites work closely with their Medicaid eligibility units to expedite the financial eligibility process for consumers. New Jersey plans to create a web-based community resource center through Harmony Information Systems.
Program Expansion and Sustainability  
Options Counseling New Jersey has developed a written handbook for options counselors and is using two new tools to assist individuals and caregivers in making informed decisions.
Care Transitions Two ADRCs are partnering with local area hospitals to provide care transition services. ADRCs are very active working with Money Follows the Person and are assiting in a variety of activities.
