VD-HCBS Quarterly Educational Calls 

Source: ACL/VHAOn December 12, 2012, the Administration for Community Living (ACL) and Veterans Health Administration (VHA) hosted an educational Webinar focused on the Veterans Directed Home & Community Based Services (VD-HCBS) Program. Topics discussed during the Webinar include the VD-HCBS Sustainability Project, VAMC Case-Mix Budget Tool and an update on the growth of VD-HCBS. A link to the presentation used during the Webinar is provided.
The second quarterly educational call was hosted on March 13, 2013. Topics discussed include program status and future directions; Veteran-directed respite care; and a presentation by Jill Snider-Meyer (Milwaukee VAMC) and Lisa Drouin (Greater Wisconsin Agency on Aging Resources) on VD-HCBS cost-effectiveness/patient satisfaction as compared with tranditional Community Nursing Home care and program expansion under the Enhanced Options Counseling (Part A) grant.
Future quarterly education calls are planned for 6/12/2013 and 9/18/2013.

Community Living Program - Program Announcement and Grant Application Instructions

2009 funding opportunity from the Administration on Aging, number HHS-2009-AoA-CD-0919. The Community Living Program (formerly known as the Nursing Home Diversion Program) helped states and communities to assist individuals at risk of nursing home placement but who are not Medicaid eligible to remain at home. AoA's collaboration with the VA continues, and states were also invited to pursue the VDHCBS option through this announcement process. Applications were due August 3, 2009.

Uniform Bill 04 (UB-04) Sample and Instructions
