The Resource Center will enable, through a single point of entry, the provision of comprehensive information, counseling, and assistance (including eligibility screening and assessment, crisis management) regarding available options and resources, and ways for individuals to plan ahead for their long-term future support. The scope of services currently being provided by the Central Mississippi AAA will be broadened, and the number of users increased through a public awareness marketing campaign, and provision of internet and telephone access. The existing MIS system will be redesigned and enhanced to provide resource, service, and evaluation information, and client tracking capabilities. |

Year I – Persons age 60+; Years II & III – Persons age 60+ and adults (age 18+) with disabilities; family caregivers and others planning for future long-term care needs will also be served |
| Streamlining Access Plans |
Current initiatives to improve access to care and service delivery undertaken by the Department of Rehabilitation Services and the Office of the Governor’s Division of Medicaid include expanding methods for service access and offering pre-admission screening for nursing care facilities or long-term placement options counseling. Meetings will be held and representation will be sought from major public providers to form formal linkages in areas such as access to nursing home Medicaid Level of Care determination processes, and Medicare and Medicaid eligibility processes. The ADRC staff will also be cross-trained to provide culturally competent, up-to-date aging, disability, and long-term support information. Both paper and web-based information and assistance packages on available care options will also be developed for clients. A comprehensive internet information service and a toll-free telephone number will be available for consumers to access ADRC services. |
