Minnesota will establish four Hennepin County/ Minneapolis resource centers in easy to access locations to provide a full complement of personalized awareness and information, assistance, and access functions. The State is developing strong linkages between county LTC consultation and information, referral, and assistance services. The State gives consumers, their families, and other caregivers and professionals access to more effective, consumer-friendly, robust decision support Internet tools to navigate LTC information for planning and decision-making. They provide standardized statewide professional screening, assessment, and eligibility using XML data sharing protocols. The State will replicate the Resource Center program throughout the state after the three-year project period. The State's goals are to have fewer low-need individuals living long-term in nursing homes, and the critical loop will be closed between assessment and care planning. |

| Streamlining Access Plans |
Using an online automated tool, consumers and staff of different resource centers have access to the same information through a web interface. Resource Center staff will offer programs and services in public places (e.g. libraries, faith-based organizations, grocery stores). |
| Program Expansion and Sustainability |
Minnesota is implementing a person-centered options counseling model statewide. |
| Options Counseling |
Minnesota has had a program of Long-Term Care Consultation (LTCC) for several years offering options counseling to all citizens regardless of income. They continue to work on integrating this with the ADRC imitative in the state. |
| Care Transitions |
As of October 2010, Minnesota had one ADRC actively conducting care transition activities. |
| Evaluation and Quality Improvement |
Evaluating and reporting the impact of the project is one of Minnesota's program objectives. |
