Minnesota Program Activities
Minnesota has received grant funding to implement an Aging and Disability Resource Center Program, a Community Living Program, and an Alzheimer's Disease Supportive Services Program. The following materials provide information on program activities in the state:
Provider Tools for Identifying and Managing Cognitive Impairment 
Source: MinnesotaThe Minnesota ADSSP grant project 90AI0021 provided funding, in part, for this six page booklet which contains: information on overcoming barriers to diagnosing dementia; a cognitive impairment identification flow chart; a provider checklist for dementia workup; and an intervention checklist for Alzheimer's Disease and other related dementia, for physicians and other health care professionals.

Minnesota Targeting Criteria and Quality Indicators for Promoting Resident Transitions from Nursing Home to Community

One of Minnesota's rebalancing strategies is to promote community discharge for persons admitted to the nursing home who wish to return to the community and can be appropriately cared for in that setting. The current report addresses the strategy by assembling research literature, analyzing nursing home length of stay and community discharge, refining community discharge targeting critieria, and developing nursing home quality indicators.
