Program Design
Overall Model Kansas has developed ADRCs in two pilot sites - one urban and one rural. The Resource Centers also have a virtual component to allow users more ease in accessing information on state and local services through a web-based resource directory of available services throughout the state, and tools to increase access to information, streamline PASRR referrals, increase mental health service access to seniors (with coordinated referral systems between AAAs and CMHC's provided), and simplify the financial eligibility determinations for Medicaid and state-funded programs (e.g., CARE II, Medikan). The goals of the Kansas ADRC project are to increase information and awareness of local resources and develop a method of providing consistent Options Counseling across agencies.
Target Populations Persons age 60+ and adults (age 18+) with mental disabilities
Streamlining Access Plans Enabling web-based virtual resource centers and the development of a web-based resource manual/directory to allow consumers to access current information on available state and local services and resources. In 1999, KDOA and SRS combined efforts to pilot test an expedited HCBS Service Delivery Model that screened clients for both medical eligibility and Medicaid financial eligibility at the same time by including an additional set of questions during the PASRR screening process, with promising results. A more streamlined, technology-focused online financial eligibility determination process will also be developed for use by screeners and consumers trying to complete the online eligibility application. Eligibility will also be streamlined for the CARE II and Medikan referral process for individuals with mental illnesses using a web-based assignment and referral system to shorten the period of time between CARE I findings and determination of the level of mental health services needed. Kansas is also working on a coordinated referral system between AAAs and CMHCs for providing a "no wrong door" approach for consumers accessing services. With their new funding, Kansas plans to streamline access to services by expediting eligibility determination.

Care Transitions As of October 2010, Kansas, a CMS Hospital Discharge Planning Model state grantee, had one ADRC actively involved in care transition activity.
