The goal of this self-assessment and planning tool is to help Aging and Disability Resource Centers (ADRCs) streamline access to services and supports. It outlines the several hoops that individuals often have to jump through to gain access to needed long-term care services beginning with the initial call. Several examples from current ADRC grantees offer context for each of the hoops about how they have improved and streamlined systems. It can be used as a tool for developing a clear strategy for streamlining access.

Streamlining Access Phone Survey Results

This document summarizes the responses provided by 23 of the 24 cohorts of 2003 and 2004 ADRC grantees during the fall of 2006 regarding streamlining access processes in their states. Grantees' responses covered the process for how financial eligibility determinations are made for Medicaid and other public LTC programs in their state, as well as how level of care assessments are conducted in their states, both before and after the establishment of the ADRC program. Other questions asked included the ADRC's role in nursing facility transitions, provision of formal options counseling to consumers, and work to intervene with critical pathways. The aggregated information provided through the streamlining access surveys has been classified and discussed according to the "fully functioning ADRC" program components and recommended metrics. The survey information collected is very helpful in providing a more detailed understanding of grantees' achievements and progress related to streamlining access.
