Program Design
Overall Model Florida has developed and implemented Aging Resource Centers (ARCs) operated by Area Agencies on Aging in planning and services areas for both publicly and privately funded services for the elderly and individuals with severe and persistent mental illness. Florida has a statewide single entry point system of ARCs, operated by the 11 AAAs in the state. Of these, four are currently ADRCs and the remaining seven are adopting plans to complete their transition to an ADRC as well. Under the ADRC Expansion Grant, Florida is now targeting the disability population of individuals with developmental disabilities in one current ADRC site and transitioning one of its aging resource centers to an ADRC. Florida pilot sites, offering a "no-wrong-door" approach, provide access to information and referral, screening and assessment, eligibility determination and options counseling. Eligibility functions are integrated through collocation of state agency staff responsible for determining both medical/functional and financial/technical eligibility. Florida pilot sites establish a single administrative structure accessible through multiple locations (senior centers, AAAs, housing authorities, mental health centers, etc.) in each of the Resource Center communities. Access to ADRC services is also available by phone and the Internet. ADRC pilot sites benefit from a recently completed state project to interface the existing client data management warehouse with a statewide Information and Referral system. Since the announcement of the 2004 ADRC grant, the Florida Legislature passed statutory changes to implement Aging Resource Centers statewide for the aging population only.
Target Populations Older adults and persons with severe and persistent mental illness. New funding will target services to persons with developmental disabilities and their caregivers.
Streamlining Access Plans Florida ADRC Pilot Sites have implemented physical or virtual collocation of eligibility determination staff. Functional eligibility is determined by CARES staff, employees of Department of Elder Affairs, who perform medical screening for Medicaid through an agreement with the state Medicaid agency. Department of Children & Families (DCF) Economic Self-Sufficiency Program does financial and technical eligibility for all public assistance programs, including Medicaid. CARES and DCF are also responsible for both functional and financial eligibility determination for the SPMI population. With new funding, Florida will streamline access to all publicly supported long-term care options including resources for persons age 50+ with developmental disabilities and their caregivers age 55+ by enhancing the existing information and referral system.
Program Expansion and Sustainability The Florida Legislature is currently funding the ARCs/ADRCs through an annual appropriation of state and Medicaid funds.
Options Counseling  
Care Transitions As of October 2010, Florida, a 2010 ACL Care Transition grantee, had one ADRC actively conducting two care transitions projects, using the Coleman Care Transitions Intervention. Other ADRCs are interested in care transitions activities and are working with Florida's QIO on implementation.
Evaluation and Quality Improvement Under the ADRC Expansion Grant, FL developed a training curriculum on developmental disabilities including supports and services. The pilot sites are using the curriculum to educate ADRC staff, members of the service provider community, local coalition workgroup and other stakeholders regarding people with developmental disabilities. They have enhanced existing information and referral system by expanding resources that may be utilized for persons with developmental disabilities. In addition, they plan to evaluate the project's effectiveness through the use of customer satisfaction surveys.
