Florida complete-screening example

This form is for a screening based on Florida's Medicaid and public assistance programs. Answer categories (instead of open-ended answers) make it easier to complete. By entering this client information into its Information and Referral (I&R) software application, the ADRC is able to share the client's responses among the agency staff assisting clients with Medicaid benefits, SNAP (Supplemental Nutrition Assistance Program, formerly Food Stamps), SHINE (Florida's State Health Insurance Program), and the Low Income Subsidy (LIS).

The answers are verified by staff as applications are completed throughout the process and the data can be updated with changes/additions when necessary. This streamlines the application process for the client since each staff member does not have to ask for the information to be repeated. The data collected are also available to the I&R Helpline and Intake staff allowing them to see what transpired, which helps them better serve the client on any subsequent calls.

Flyer for Florida's Alternative Transportation Options

The University of Florida’s Institute for Mobility, Activity and Participation developed the Florida Senior Safety Resource Center (FSSRC), a database containing over 700 transportation services. This user directed information for seniors, or persons with disabilities, addresses the need to find appropriate, acceptable and affordable transportation within their local communities. These services range from highly organized to very flexible and provide various levels of assistance, throughout all of Florida’s 67 counties.

Because the database is owned collectively by the AAAs, the State Unit on Aging does not have a role in setting standard inclusion and exclusion policy, but requires each site to have individual inclusion and exclusion policies consistent with the Alliance for Information and Referral Systems (AIRS) standards.

Screening instrument for determining an individual's priority for aging services so that those in greatest need and with the least assistance available will receive services first. This screening is usually completed over the telephone. Even though targeting criteria differ between the programs, both Florida’s CLP and ADRC Care Transition programs use this form to gather financial information for eligibility screening.

Protocols recently modified for Florida's 2008 CLP grant award project. These include protocols on identifying consumers, the initial consumer assessment and screening consumers for functional and financial eligibility. States implementing Care Transition programs can learn details about Florida’s process for screening individuals’ functional and financial status. States can also learn from Florida’s use of income and asset screening as well as the use of income/asset minimums and maximums as financial targeting criteria.

Florida Marketing Plan for ADRC

Florida Department of Elder Affairs Prioritization Assessment Form

This document, also called the 701A, is used statewide in Florida, not specifically for the ADRC initiative. Florida is in the process of developing a new Eligibility Screen instrument for testing by the ADRCs, which will be an expanded version of the current 701A, and will collect additional information to streamline the eligibility determination referral process for those appearing to be eligible for public assistance programs, such as Medicaid and Food Stamps. The new instrument is being designed in the format of a script to be completed electronically through interview between a trained ADRC professional and the applicant. Until the new Eligibility Screen is implemented, the 701A will continue to be used in the three ADRC pilot sites.

The State of Florida is vulnerable to a variety of hazards that threaten its citizens, communities, businesses, economy, and the environment. The Continuity of Operations (COOP) Plan establishes policy and guidance to ensure the execution of the State of Florida, Department of Elder Affair's (DOEA) AAA mission essential functions in the event that mission services are threatened or incapacitated, and the relocation of selected AAA/ARC personnel and functions is required.
