2012 ADRC Joint Program Official Funding Announcement

This opportunity is for states to significantly strengthen and expand their long term services and supports Options Counseling (LTSS OC) programs. Application deadline is July 25, 2012.

States serve a substantial number of people with dementia and their family caregivers in their Aging Networks and LTSS systems. Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

Piedmont Triad Expansion Manual

The Piedmont Triad ADRC Guidebook provides information to assist ADRCs — called Community Resource Centers (CRCs) in North Carolina – as they develop their programs and begin to offer services and supports to seniors and people with disabilities under the auspices of the state CRC initiative. The guidebook contains eight sections with content ranging from an overview of the ADRC initiative, the organization and operation of CRCs in North Carolina, to marketing, training, referral processes, etc. Most sections, as well as the appendices, provide resources, sample documents and tools for emerging CRC programs. The guidebook is a good model for how to organize and present orientation materials to new, emerging ADRCs, especially those operating as “No Wrong Door decentralized programs.
