Connecticut Program Activities
Connecticut has received grant funds to implement an Aging and Disability Resource Center Program and a Community Living Program. The following materials provide information on program activities in the state.




Connecticut VD-HCBS Materials 
Source: ConnecticutConnecticut created the following resources to assist with the VD-HCBS program: (1) VD-HCBS Fact Sheet to collect information about the consumer; (2) VD-HCBS Budget and Plan of Care Development Process document; (3) Rainy Day Fund Accruals FAQ sheet; (4) VD-HCBS Billing Process Summary; and (5) Veteran Outreach TriFold Brochure. Connecticut recently expanded its VD-HCBS program to a second region.

Connecticut's Long Term Support Options Guide

This support guide provides information regarding long term care and housing options for consumers in options counseling. The guide covers home modifications, long term care and long term care financing (both private and public), alternative housing options, reverse mortgages, and several other topic areas. The guide includes a financial planning worksheet, a checklist for staying healthy, and a worksheet for organizing personal documents and information.

Connecticut - CT Evaluation Tool

Referred to as the “Care Transitions Challenges / Barriers Measure.” It was adapted from a checklist used by the CT Money Follows the Person Program and is used by Care Transition Coaches to record all observed barriers at any point during the intervention.

Online Caregiver Training

Through this interactive training, family caregivers can learn about available resources and train themselves on how to deal with common challenges. Some of the resources featured are links to agencies and senior centers, care locator tools, and descriptions of helpful programs. Trainings cover how to recognize and prevent stress, improve communications with family members and service providers, and address conflicts.
