| Overall Model |
California has implemented ADRC partnerships in eight areas of the state. The core partnership is between an AAA and an ILC with focus on streamlining consumer access to information and services. CA uses a no-wrong-door approach to the ADRC model. |

| Streamlining Access Plans |
Consumers experience streamlined access to information and services through better coordination, reducing duplication and sharing information across networks. Partnerships between AAAs and ILCs and other health care social service organizations are being promoted and where possible, supported. |
| Program Expansion and Sustainability |
The State Independent Living Council and the Health and Human Services Agency will be examining the feasibility tools for developing statewide ADRCs. Additionally, designation standards for ADRC and standards for Options Counseling is included in work under a grant under the Affordable Care Act. |
| Options Counseling |
As a 2010- 2012 Options Counseling grantee, California is collaborating with the Administration for Community Living (ACL) to create national minimum standards for options counseling as well as developing statewide standards. Draft state standards will be tested in at least one area beginning in October 2011. CA is in the process of surveying consumers to gauge the effectiveness of the options counseling program. |
| Care Transitions |
As of October 2010, California, a CMS Hospital Discharge Planning Model and 2010 AoA Care Transitions grantee, had four ADRCs actively conducting care transitions activities and one ADRC currently planning to implement care transitions. All sites are either currently using or plan to use the Care Transitions Intervention. |
