Program Design
Overall Model AZ Links, Arizona's ADRC employs a no wrong door approach to provide information, referral and access to services through the members of its AZ Links Consortium. Key partners include the Arizona Department of Economic Security, Division of Aging and Adult Services (ADES-DAAS) and Division of Developmental Disabilities (ADES-DDD), Area Agencies on Aging, and the Arizona Health Care Cost Containment System (AHCCCS). Regional AZ Links Partnerships involve additional partners, such as Centers for Independent Living and community providers, also integrating existing programs such as Benefit Check-Up Arizona, State Health Insurance Assistance Program, Older Americans Act's home and community based and caregiver programs, and Medicaid long-term care services. The central website, www.AZLINKS.gov, serves as a virtual resource center where individuals can access information through a self-help web portal and find community AZ Links partners for counseling and assistance on long-term support options. Arizona currently has six regional partnerships, covering all 15 counties (excluding tribal areas). These partnerships conduct networking meetings and training sessions for their regional partners, using a common reference manual.
Target Populations Persons age 60+ and adults (age 18+) with physical or developmental disabilities, caregivers, and professionals.
Streamlining Access Plans All Area Agencies on Aging use standardized assessment tools. One regional AZ Links partnership is piloting an effort to utilize a common intake form for its regional partners. The AZ Links Steering Committee continues to discuss options for improved collection of client data and streamlining access.
Program Expansion and Sustainability Arizona is looking into the possibility of starting Tribal ADRCs as part of their goal of expanding access statewide. AZ Links is also participating in a discussion with Community Information & Referral (CIR) and Arizona United Way to develop a strategic plan for collaboration with the 2-1-1 service (website and telephone) for easy access to information about and referrals to health and humans services statewide.
Options Counseling As a 2010- 2012 Options Counseling grantee Arizona is collaborating with the Administration on Aging to create national minimum standards for options counseling as well as developing statewide standards. Options counseling is being offered in Maricopa County and Pima County at the Area Agencies on Aging and the Centers for Independent Living.
Care Transitions Arizona has four ADRCs actively conducting care transitions activities.
Evaluation and Quality Improvement AZ Links partners use satisfaction surveys and internal reviews to evaluate programs. For special programs, such as Care Transitions and Options Counseling, AZ Links partners are creating common tracking tools.
