The National Resource Center on LGBT Aging is the country's first and only technical assistance resource center aimed at improving the quality of services and supports offered to lesbian, gay, bisexual and transgender (LGBT) older adults. Established in 2010 through a federal grant from the U.S. Department of Health and Human Services, the National Resource Center on LGBT Aging provides training, technical assistance and educational resources to aging providers, LGBT organizations and LGBT older adults. Providing Services for People with Dementia Who Live Alone: Issue brief

Serving this population effectively involves accommodating the needs of a population that, in addition to memory loss, experiences a variety of physical, cognitive, and behavioral symptoms resulting from dementia, along with other medical conditions. In model dementia-capable systems, programs are tailored to the unique needs of people with Alzheimer’s disease or other dementias, and their caregivers.

South Carolina Hospital Discharge Planning Program Process Enhancement

In October of 2008 South Carolina Lt. Governor’s Office on Aging was awarded an approximately $1.3M grant by Center on Medicare and Medicaid Services (CMS) to study, develop, and evaluate an ideal person-centered discharge planning process. The South Carolina Person Centered Discharge Planning (SC-PCDP) team was established including LGOA, the Appalachia Area Agency on Aging, University of South Carolina (USC), Community Long Term Care (CLTC), Spartanburg Regional Hospital System (SRHS), and other agencies and community stakeholder organizations. The team received person-centered training and created a core process committee to begin the study and development effort.

Moving Toward Person- and Family-Centered Care

This Insight on the Issues from the AARP Public Policy Institute defines Person- and family-centered care (PFCC), highlights the importance of the family in a PFCC approach, and describes the key elements of PFCC in practice. Although research shows that PFCC improves quality of care and quality of life, it has not yet been fully integrated across health care and LTSS as an essential part of all care and support. However, the current shift in payment reform and service delivery away from disease or provider-focused models and toward incentives that reward better coordination, communication, and continuity of care, is likely to promote wider adoption and effective implementation of PFCC.
