Contact Us! adrc-tae@lewin.com adrc-tae.org Fall 2011 SART Open The Fall 2011 Semi-Annual Reporting Tool is now open for reporting throughout the month of October. Data reported by grantees through the SART help provide a fuller picture of all the program activity across the nation and in each state. Thank you in advance for your effort in providing full and comprehensive reports about your activities and sharing lessons learned so they might benefit the larger grantee community. Two SART trainings are scheduled in October. Attending one of these trainings is recommended for program staff reporting throug the SART for the first time. We encourage project directors to encourage everyone on your teams who may contribute to the reports to attend, for example staff that support care transitions, options counseling, and MFP/ADRC coordination activities, so they are familiar with the kinds of questions and information asked about in the report. Patient Centered Outcomes Research Funding Opportunity The Pilot Projects Grant Program, is a new major funding opportunity from the Patient-Centered Outcomes Research Institute (PCORI). The Institute intends to support 40 awards with up to $26 over two years. PCORI wants to fund projects working to develop research methods, patient-oriented outcomes instruments, patient-provider communication and other decision- making strategies, build collaborative research teams with stakeholders, translate research findings into clinical practice, stakeholder engagement, and research agenda setting strategies that can be used in future comparative effectiveness research. Projects should be focused on furthering Patient Centered Outcome Research. Application materials are available from the PCORI Website. New Resources CMS hosted a call about "MDS 3.0 Section Q Opportunities and Challenges One Year After Implementation" on September 28, 2011. The call included: information about the impact of Section Q referrals and related activities of the Money Follows the Person (MFP) and Aging and Disability Resource Centers (ADRC) Grantees; States experiences with implementation challenges and successes; MDS 3.0 Section Q changes for implementation April 2012; and National Section Q training being planned for late February 2012. Resource materials and discussion also addressed how State Veterans Homes (SVHs), MFP and ADRC Grantees, and local contact agencies (LCA) can coordinate efforts to assist Veterans who want to transition back to the community. The Agenda, Powerpoint Presentation, and Proposed Language Pilot Test Report have all been posted. New York's Care Transitions Flow Chart displays the pathways of their care transitions program operations and processes. Discussion Forum Update The original poster of a question about Benchmarks for Call Handling posted thank everyone who provided information in response. It's a great example of how the discussion forums can work! The discussion forums exist to facilitate active, informal, peer exchange. Consider using the forums to seek or offer help, advice, and input, or to extend discussion after a webinar.
