Welcome to the new and improved ADRC Readiness Assessment. This assessment tool has been updated as of November 2012 to reflect the current federal expectations for ADRCs outlined in the 2012 ACL/CMS/VA Joint Program Announcement for states to expand and enhance their ADRC Options Counseling Programs. The Lewin Group designed the original version of this assessment in 2007. Its purpose is to assist organizations at the state or local level to evaluate their readiness to perform key functions of Aging and Disability Resource Centers (ADRCs). The assessment tool is:

     

  • Easy to use--multiple choice questions ask you to describe the present activities, experience and status of your organization or coalition of organizations
  •  

  • Confidential--all information is password protected and available only to the user
  •  

  • Informative--assesses organizational readiness in seven functional areas of ADRCs and many of the questions  include links to additional information or resources about those items
  •  

  • Responsive--results are evaluated and immediate feedback is provided
  •  

  • Convenient--the survey takes about 30 minutes to complete and, if necessary, you can save your work and come back later.

 

Three Easy Steps

 

You can complete the survey and obtain your ADRC Readiness Profile in three easy steps:

 

Step I --  Provide basic background information about your organization (or coalition of organizations) and create a unique password. This will ensure the confidentiality of your responses and will enable you to save your survey and return to it in the future, if needed.  The password you create will also be required to access your ADRC Readiness Assessment Results which will appear after you have submitted the completed survey for analysis.

 

Step II -- Complete the ADRC Readiness Assessment Survey. 

The questions in the Readiness Assessment Survey are organized into seven main sections. You may start with any section below and return to the Survey Home Page to complete the other sections or start with Section 1 and continue through the entire survey in order.

Throughout the assessment, some of the questions contain underlined words that link to their definitions.

Step III— Review your responses and submit your completed survey for analysis.

 

After the completed survey is submitted, your ADRC Readiness Assessment Results will appear on the screen. These Results will detail your organization's level of readiness in the main areas covered in the survey and offer links to resources that may help your organization build capacity in these areas. You should print your Readiness Assessment Results, but they will also be saved in the system and you may access them again online by using the unique username and password you create at the beginning (in Step I above).
